Insurance
I am a participating provider in most insurance plans, including Medicare. At the present time I am an in-network provider for the following:
If I am not a participating provider in your insurance plan your health insurance carrier may still allow you to choose an out-of-network provider, and the services may be covered in full or in part by your health insurance or employee benefit plan.
Prior to calling for an appointment, it is suggested that you contact your insurer to determine the following:
Do you have mental health benefits?
Am I a participating in-network provider?
If I am not a participating provider, how are out-of-network services covered?
How many sessions per year are covered by your insurance plan?
Is there a "deductible" that you will be responsible for before insurance payments?
What is the coverage amount and co-pay for each session?
Is pre-authorization or a referral required?
Fees
Fees will be discussed when you call for your initial appointment.
Insurance deductibles and/or co-pays are due and collected at the time of your visit
If you have insurance we will process the claims for the balance. Claims denied by the insurance company will remain your responsibility for payment
